WHO urgently requests additional clinical and treatment data from the regions and countries where patients infected
with the new influenza A (H1N1) virus are being treated to improve understanding of the disease and refine optimal case management. Reporting clinical findings and treatment outcomes to WHO will greatly facilitate the better understanding of the new disease and the development of further management guidance.
Here we provide you with a form which is issued by The WHO in order to obtain important information to determine severity and clinical characteristics of the cases infected with “new” Influenza A(H1N1). Within this form, there are sections of reporter information, case information, location on symptoms onset, laboratory test results, symptoms, hystory and preexisting conditions, possible exposure, outcome, symptoms occuring at anytime during the occure of the disease, developed pneumonia, treatment provided, complications during the course of disease, and other comments/observations.
